Registration Schedule for December NLE 2011 Passers-Manila & Pampanga

 

 

The December 2011 Nursing Licensure Examination (NLE) passers registration schedule in Manila and Pampanga are already release by the PRC. Initial registration will start this February 27 to March  29,2011 at the PRC Auditorium, 5th Floor, Annex Building from 8:00 Am to 5:00 pm.Here are the Registration Schedule of Registered Nurses.

 Mar  1 GERONIMO, ANNA MIYET L. – MAGALLANES, JENNE MAY A.
 Mar  2 MAGALONA, RUBIE LYN S. – PAILAN, KAMIL RACHEL L.
 Mar  5 PAJA, RENO B. – SANTOS, SHIELA MARIE B.
 Mar  7 SANTOSIDAD, KRISTIFFANY B. – ZUNIGA, MARK DAVID B.

Those fail to register on their scheduled date may register on the following dates:

 Mar 8 AALA, LALLANE S. – LACESTE, KAREN ANN M.
 Mar 9 LACHANOA, ABEGAIL B. – ZUNIGA, MARK DAVID B.
Requirements
  • Duly accomplished Oath Form or Panunumpa ng Professional (see below)
  • Two (2) pieces passport size colored picture in white background with complete name tag (lastname, first name, middle name) e.g. DELA CRUZ, JUAN M.
  • One (1) piece 1×1 colored picture in white background (same shot)
  • Community Tax Certificate (Cedula) [Should be dated this year 2012]
  • Payment: Registration Fee (Php 600.00)/ Annual Fee (Php 450.00)
  • Metered Documentary Stamp (available at PRC offices)
  • One (1) short brown envelop with name and profession

Nurses are advice to come early and prepare the necessary requirement a day before going to the site. Don’t forget to eat and bring a bottle of water.  It long line of standing you might end up skipping your meal just to complete the registration. Good Luck Fellow Nurses… 🙂
